Antonio Community Hospital participates in a number of local collaboratives
focused on improving the health and welfare of our community. A brief
description of some of the collaborative efforts follows. For more information

Upland Healthy Start Healthy Future Collaborative
This collaborative was instrumental in the planning and development
of Upland's Healthy Start Program, which provides integrated services
for children and their families at four elementary schools and one junior
high school in the City of Upland. S
ervices
range from providing basic necessities, such as food, clothing, and
health care, to parenting and citizenship classes.
Family Solutions Collaborative
Family Solutions is a proactive neighborhood-based collaborative supporting
the Family Solutions Centers. The centers, located in central and southern
Ontario, provide counseling, tutoring/mentoring, parent education, job
readiness training, and health services for students and their families
at two preschools, six elementary schools, and two junior high schools
in Ontario.
Focus West
The Focus West collaborative includes organizations and agencies providing
services for youth throughout western San Bernardino County. Focus West
has served as a catalyst in coordinating efforts among various organizations
to reduce duplication and gaps in the services available for children
and teens.
Montclair Community Collaborative
The Montclair Community Collaborative is a working partnership, which
includes the City of Montclair, Ontario-Montclair School District, San
Bernardino County agencies, healthcare providers, and nonprofit organizations,
whose focus is to improve the community of Montclair. The Collaborative
has successfully improved the delivery of services by reducing duplication
and filling in the gaps. Several major grants have been awarded to the
Collaborative, which have provided resources to develop new and expanded
services including a community health clinic, Por La Vida's health and
wellness educational program, and the Online to College program for
students attending Montclair schools.
Upland Community Collaborative
The Upland Community Collaborative is a group
of concerned individuals working cooperatively to improve the community of Upland.
The goal is to meet the changing needs of all Upland residents, especially children,
families, senior citizens, or others with special needs, by building on the assets
and strengths of the community. Specific objectives have been established, which
include providing access to basic human needs like food, clothing, shelter, and
healthcare; maintaining safe and stable neighborhoods; and providing educational,
cultural, and recreational opportunities for all members of the community, from
youth to seniors.
